Compensation & Schedule

  • Paid per visit position – routine visit rates starting at $65 per visit

Position Summary

We’re seeking a compassionate and motivated Medical Social Worker (MSW) to join our Home Health team on a PRN basis in the Dublin, Ohio area.
The MSW will evaluate and support patients and families dealing with medical, emotional, and social challenges — helping them access the resources and support needed for successful care at home.


Key Responsibilities

Provide psychosocial evaluation, diagnosis, and treatment planning
Offer individual, family, and/or group therapy as needed
Assess and address social or personal issues impacting patient independence and recovery
Connect patients and families to appropriate community resources
Collaborate with physicians, nurses, and interdisciplinary care teams
Maintain accurate, up-to-date documentation per HIPAA standards
Provide written reports for patient records, including evaluations, progress notes, and discharge summaries
Participate in family conferences and patient care planning
Discuss care concerns and recommendations with the Director of Clinical Services

Qualifications

Master’s degree in Medical Social Work (MSW) or equivalent license
Minimum 2 years of experience in a medical or home health setting
Current CPR certification
Valid driver’s license and proof of auto insurance
Ability to attend a 1–2 day orientation at our Dublin office
