• Conduct comprehensive patient intakes, emphasizing diagnostic and functional evaluations to establish treatment goals.
• Provide brief, focused therapeutic interventions tailored to patients seeking mental health support.
• Advise primary care providers on the suitability of patient care within the clinic versus referrals to specialty mental health facilities.
• Monitor patient progress through follow-up assessments, making necessary adjustments to treatment plans.
• Develop individual relapse prevention plans to support patients in maintaining mental stability.
• Document patient progress and diagnostic information diligently in treatment charts, complying with agency standards.
• Collaborate closely with primary care teams to ensure comprehensive treatment plans that address both mental and physical health needs.
• Assist patients in navigating healthcare services, linking them with additional treatment providers as required.
• Educate patients on self-management strategies, equipping them with the tools necessary for active participation in their recovery.
• Provide consultations to clinic management and team members on behavioral services, contributing to program evaluation and outcomes.

The LPC will work with a diverse client population, including individuals facing various mental health challenges such as anxiety, depression, trauma, and other psychosocial issues. • Master’s Degree in Clinical Counseling or a related field.
• Valid and current Licensed Professional Counselor (LPC) credential in Texas.
• Thorough understanding of mental health assessment, treatment planning, and various therapeutic approaches including, but not limited to, family and group therapy techniques.
• Excellent communication skills, with the ability to establish rapport and trust with a diverse patient base.
• Strong organizational skills and proficiency in maintaining accurate clinical documentation.
• Ability to comply with the Code of Ethical Conduct for Clinical Professionals.

• Experience working in a community health setting, particularly with Federally Qualified Health Centers (FQHC).
• Familiarity with DSM-IV diagnoses and psychotherapeutic methodologies.
• Bilingual proficiency in English and Spanish is a significant plus, enhancing communication efforts with a broader clientele.
