Job description

Case Manager

Duties

  • Assess clients’ needs through detailed interviews and comprehensive evaluations, identifying immediate risks and long-term goals.
  • Provide crisis intervention and crisis management support during urgent situations, ensuring client safety and stability.
  • Utilize motivational interviewing techniques to motivate clients toward positive behavioral changes and sustained engagement with services.
  • Offer addiction counseling support when appropriate, guiding clients through recovery pathways and relapse prevention strategies.
  • Maintain thorough documentation of all client interactions, service plans, progress notes, and incident reports in compliance with organizational policies.
  • Collaborate with community agencies, child welfare organizations, and housing authorities to ensure seamless service delivery.

Skills

  • Strong crisis intervention skills with the ability to remain calm under pressure and manage urgent situations effectively.
  • Expertise in addiction counseling techniques coupled with a solid understanding of behavioral health principles.
  • Proven experience in crisis management and conflict resolution within social service settings.
  • Deep knowledge of social work practices, including client advocacy and case planning.
  • Proficiency in motivational interviewing to foster client motivation and engagement.
  • Knowledge of workforce development initiatives aimed at employment readiness and skill-building for diverse populations.

Group Facilitator

Duties

  • Facilitate structured group therapy sessions utilizing evidence-based approaches.
  • Lead discussions on topics including grief, trauma (including PTSD), addiction recovery, gender issues, chronic pain management, and behavioral management strategies.
  • Create a welcoming environment that encourages open communication while maintaining confidentiality in accordance with HIPAA regulations.
  • Monitor participant progress through ongoing evaluation and adjust facilitation techniques accordingly to maximize therapeutic outcomes.
  • Document session notes accurately within medical records.
  • Collaborate with interdisciplinary teams including social workers, psychologists, psychiatrists, and case managers to coordinate care plans.
  • Manage crisis situations effectively by implementing crisis intervention protocols and de-escalation techniques when necessary.

Requirements

  • Bachelor’s degree in social work, psychology, counseling or related field; degree preferred. CDCA REQUIRED
  • Proven experience facilitating group therapy or support groups in clinical or outpatient settings.
  • Experience with crisis management techniques and behavior management strategies across diverse populations.
  • Proficiency in medical terminology and healthcare documentation standards including HIPAA compliance.
