• Comprehensively evaluate and treat patients in-home settings who are recovering from injuries, disease, or surgical procedures.
• Develop and implement personalized treatment plans that utilize a variety of methods, including exercise, therapeutic modalities, and education, to alleviate pain and enhance mobility.
• Monitor progress and document treatment responses, ensuring that patient records remain accurate and up-to-date.
• Reevaluate treatment plans periodically to adapt to patients’ evolving needs and redefine therapeutic goals.
• Provide guidance on assistive devices, ensuring proper fitting and training for patients and their caregivers.
• Engage with patients and their families, educating them about therapy options, self-care techniques, and necessary follow-up procedures.
• Maintain professional development by attending workshops, reviewing literature, and participating in relevant professional organizations.

In this role, you will work with a diverse patient population that includes children, adults, and seniors with varying physical therapy needs. From post-surgical recovery to chronic pain management, you will tailor your approach to meet the unique challenges and goals of each individual. Your contributions will not only support patient recovery but also empower them to regain independence and maintain a healthy lifestyle.

• Bachelor's Degree in Physical Therapy required; a Doctorate in Physical Therapy (DPT) is preferred.
• Active State License in Physical Therapy.
• Strong verbal and written communication skills to effectively interact with patients and families.
• Proficiency in basic computer usage and documentation software is essential.
• A commitment to ongoing personal and professional growth.

• A minimum of 2 years of experience in a clinical setting, preferably in home care.
• Experience working with assistive devices for rehabilitation.
• Familiarity with documentation protocols and electronic medical records.
