Care Companion/Social Worker - Oshkosh, WI

Carease Health - WI is seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team in Oshkosh, WI. In this rewarding part-time role, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

About the Role:

As a Care Companion/Social Worker, you'll play a vital role in enhancing the well-being of older adults in long-term care settings. You'll conduct assessments, implement short-term therapy and supportive interventions, and collaborate with psychiatric professionals to ensure comprehensive care. Your visits will be scheduled according to a set schedule with your supervisor, and you'll maintain detailed records of interactions and interventions after each visit.

Key Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

• Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
• Education: Associate Degree
• Experience: 1 year
