We are seeking qualified Nurse Practitioner to service patients in the North Augusta area. As a Provider, you play a critical role in delivering exceptional patient care in patients' homes. You are an integral part of the care team, collaborating with multiple healthcare professionals to ensure comprehensive and effective patient management. Your responsibilities include evaluating high-risk patients, coordinating care plans, and providing care management to improve patient outcomes.
Our Provider role performs visits in their designated service area. This is a full-time salary-based position, with the opportunity to receive a monthly bonus.
