Position Overview

Crimson Care Center is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic team. This full-time remote position provides a unique opportunity to contribute to the lives of patients and their families as they navigate the complexities of hospice care.

Work Environment

The position offers a fully remote work setting, enabling you to provide critical support while maintaining work-life balance. Although your role will primarily involve virtual interactions, you will play an integral part in our hospice care team, collaborating with medical professionals and support staff dedicated to improving patient outcomes.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to formulate independent diagnoses and develop effective, individualized treatment plans.
  • Utilize evidence-based therapeutic techniques such as solution-focused therapy, motivational interviewing, and behavioral activation to address patientsโ€™ emotional and psychological needs.
  • Engage in regular collaboration with interdisciplinary teams, including medical doctors, nurses, and other clinicians, to ensure an integrated care approach tailored specifically for hospice patients.
  • Monitor and document treatment responses, adjusting plans as necessary to promote ongoing progress toward established goals.
  • Support the management of psychotropic medications, ensuring adherence and assessing side effects within the context of hospice care.
  • Facilitate support groups and provide individual counseling to patients and their families, helping them cope with the emotional aspects of end-of-life care.
Patient Population / Client Focus

In this role, you will primarily work with adults facing terminal illnesses and their families, offering compassionate support during challenging times. Your expertise will be vital in assisting patients to manage their emotional responses, facilitate discussions about end-of-life issues, and enhance their overall quality of life during hospice care.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • Possession of a current, valid, and unrestricted LCSW license to practice in the State of New York.
  • A minimum of one year of experience as an independently licensed therapist, preferably within a mental health or healthcare environment.
Preferred Experience
  • Experience in hospice and palliative care settings is highly desirable.
  • Familiarity with telehealth practice and digital tools for mental health support.
  • Strong communication and interpersonal skills to effectively engage with patients and their families.
Compensation and Benefits

The salary range for this position is competitive, offering a salary between $80,000 and $100,000 per year, commensurate with experience and qualifications.
