• Conduct thorough clinical assessments to evaluate the emotional and psychological needs of patients and their families.
• Develop tailored treatment plans and therapeutic interventions that address specific patient challenges and enhance their quality of life.
• Implement solution-based therapeutic practices, utilizing evidence-based strategies to empower patients and families during their journey.
• Document patient interactions, treatment plans, and outcomes in a timely manner using our secure electronic health record system.
• Collaborate with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to coordinate holistic care for patients.
• Provide education and resources to patients and families regarding adjustative strategies, coping mechanisms, and available support services.
• Remain updated on the latest advancements in mental health research and explore innovative practices that can improve patient outcomes.

You will primarily work with patients who are receiving hospice care, often facing terminal illnesses such as cancer, organ failure, or dementia. The role entails not just addressing their psychological needs, but also providing crucial emotional support to families, helping them navigate grief and decision-making during difficult times. Your expertise will directly enhance the patient experience and bolster their dignity and comfort during their end-of-life journey.

• Must possess a valid LCSW license in the state of Tennessee.
• A Bachelor's Degree in Social Work or relevant field is required, although candidates with a Master's degree are preferred.
• Strong understanding and experience in solution-based psychotherapy interventions.
• Exceptional communication, interpersonal, and problem-solving skills to effectively engage with patients and families.

• A minimum of 2 years of clinical experience working in a healthcare setting, preferably in hospice, palliative care, or end-of-life issues.
• Experience with electronic health records and telehealth platforms to facilitate virtual patient interactions.
• Familiarity with interdisciplinary team dynamics in a healthcare environment.
