Gotham Enterprises Ltd is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our remote team as a full-time Social Worker dedicated to hospice care. Work Environment

This position is entirely remote, allowing for flexibility while ensuring that you can efficiently serve seniors who require hospice care from the comfort of their homes or residential facilities. While our main facility is located in Chattanooga, Tennessee, the role allows you to operate effectively from any location. You will follow a structured full-time schedule from Monday to Friday, typically between the hours of 8:00 AM to 4:30 PM, with occasional evenings or weekends scheduled as necessary to meet the needs of clients and ensure continuity of care.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to understand the emotional and social needs of seniors in hospice care.
  • Develop personalized care plans in collaboration with patients and their families, focusing on individual goals, preferences, and resources.
  • Provide ongoing counseling and emotional support to clients and their caregivers, assisting them in navigating the complexities of end-of-life planning and transitions.
  • Advocate for clients' rights to access necessary community resources and services, ensuring they receive timely and appropriate assistance.
  • Engage with interdisciplinary teams including nurses, medical professionals, and caregivers to coordinate holistic care tailored to each clientโ€™s unique situation.
  • Regularly visit clients to monitor their emotional and physical well-being, ensuring that their living conditions are safe and supportive.
  • Facilitate communication and conflict resolution among family members, promoting harmonious care environments.

  • Maintain accurate and timely documentation of client assessments, care plans, and interactions in compliance with ethical standards and regulations.

Qualifications
  • Master's degree in Social Work from an accredited institution is required.
  • Possession of current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of relevant experience in a social work capacity, ideally in hospice or geriatric settings.
  • Extensive knowledge of social work principles, laws, and available community resources related to senior care.
