Palliative Care Advanced Practice Provider - Hospital Based


Location: Ascension St. Thomas Midtown, Nashville TN

Status: Full-Time | Monday-Friday | 40 hours/week 

Reports to: Vice President, Clinical Operations & Palliative Care


About This Role

Join Alive, Middle Tennessee's original hospice and a trusted leader in end-of-life care for 50 years, as a Palliative Care APP embedded at Ascension Saint Thomas Midtown. You'll serve as an inpatient palliative care consultant, helping patients and families navigate complex illness through expert symptom management, goals-of-care conversations, and advance care planning. This is an opportunity to build meaningful relationships with a single hospital team, grow your own referral base, and practice at the top of your license with the backing of a respected, mission-driven organization.

What You'll Do

  •  Provide palliative care consultations alongside hospital medical teams, managing symptoms related to acute and chronic   conditions
  •  Lead compassionate conversations with patients and families about goals of care, prognosis, and treatment options
  •  Develop and implement individualized care plans in collaboration with physicians, social workers, chaplains, and case  managers  
  • Build and nurture referral relationships within your assigned hospital
  • Order and interpret labs, imaging, and medications; document thoroughly in the medical record
  • Serve as a resource and mentor to students, residents, and new team members

Requirements


What You Bring

  • Graduate of an accredited NP, CNS, or PA program
  • Active, unrestricted Tennessee license and national certification
  • Unrestricted DEA
  • Valid driver's license and insurance

Preferred:

  • Experience in hospice, palliative care, ICU, or inpatient acute care
  • ACHPN or HPCC certification (or willingness to pursue)
