• Conduct ongoing assessments to develop individualized treatment plans tailored to the specific needs of each patient.
• Communicate regularly with supervisors, as well as family members and guardians, promoting collaborative care focused on the patient’s progress.
• Organize and coordinate home visits, ensuring compliance with in-home observations and facilitating comprehensive assessments through chart reviews and staff interviews.
• Participate actively in Interdisciplinary Team Meetings, contributing insights that enhance care strategies and improve patient outcomes.
• Maintain meticulous records of patient progress and treatment plans to ensure continuity of care and availability of information to relevant stakeholders.
• Provide ongoing training to staff members on program documentation and implementation strategies, ensuring high-quality service delivery.
• Act as a liaison with the public and various mental health communities, promoting development goals that align with our organizational mission.

The Licensed Professional Counselor will work with a diverse range of clients, including adults and youth facing a variety of mental health challenges. Our patient population may encompass individuals dealing with anxiety, depression, trauma, substance misuse, and family dynamics. Your role will prioritize individualized care, leveraging your skills to navigate the complexities of mental health and fostering strong therapeutic relationships with clients.

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.
• An active, unrestricted license as a Professional Counselor (LPC) in Oregon is mandatory.
• At least two years of relevant counseling experience, with familiarity in clinical settings preferred.
• A deep understanding of legal and ethical standards within the mental health field is vital to this role.
• Excellent interpersonal and communication skills, with the ability to establish rapport with a variety of clientele and collaborate effectively with healthcare teams.
• Experience utilizing EMR software and other relevant technologies to enhance patient care and documentation processes.

• Specialization or significant experience in family therapy, couples therapy, trauma-informed care, or substance misuse is highly preferred.
• Familiarity with evidence-based practices and emerging trends in mental health treatment, aiding in the continual improvement of our service offerings.
• Proficiency in conducting home visits and assessments can be an advantage.

• Your salary will range from $90,000 to $100,000 annually, commensurate with your experience and caseload availability.
