This position operates in a hybrid model, allowing for both remote work and outpatient office interactions when necessary. As a Licensed Professional Counselor, your expertise will be vital in enhancing the mental health and wellness of our clients. Your duties will include:

• Conducting comprehensive assessments to tailor treatment plans that address the unique needs of each patient.
• Maintaining open lines of communication with supervisors, colleagues, and families to report on patient progress and effectively coordinate care.
• Developing a clear schedule of appointments and home visits, ensuring consistent and proactive follow-up for ongoing care.
• Coordinating in-home observations and reviews to provide holistic insights into patient behavior and progress.
• Implementing and monitoring program plans to ensure they are executed effectively, facilitating optimal patient outcomes.
• Participating in interdisciplinary team meetings to collaborate on patient care strategies and share insights and findings.
• Documenting monthly and quarterly progress notes to track patient development and guide future treatment approaches.
• Organizing training sessions to ensure all staff members are equipped with the knowledge and skills necessary to implement program requirements effectively.
• Providing follow-up care and support through interviews and additional home visits to ensure continuous improvement of client welfare.
• Acting as a liaison with community resources to promote awareness and support for mental health initiatives.

Our counselors work closely with a diverse clientele, including adults and youth facing various mental health challenges. You will have the opportunity to provide tailored support for conditions such as anxiety, depression, trauma, and substance misuse, with a focus on building healthy relationships and fostering personal growth.

To be considered for this rewarding position, candidates must meet the following qualifications:

• Master's or Doctoral degree in Counseling, Psychology, or a related field.
• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona.
• Minimum of two years of counseling experience in a clinical setting, demonstrating a strong foundation in therapeutic practices.
• Deep understanding of legal and ethical standards that govern the practice of counseling.
• Exceptional interpersonal skills to build rapport and trust with clients, families, and colleagues.
• A collaborative spirit, showcasing a proven ability to work effectively within a multidisciplinary healthcare team.
• Proficient in the use of Electronic Medical Records (EMR) systems and technology to support patient documentation and communication.

While not mandatory, applicants with experience in family and couples therapy, as well as specialties in trauma-informed care or substance misuse, will be highly regarded. Your unique skills and experiences can contribute significantly to the richness of our counseling team.

The annual salary for this position ranges from $90,000 to $100,000, commensurate with experience and client caseload. In addition to a competitive salary, we offer:
