What You'll Do

Provide compassionate, hands-on care that supports patient comfort, safety, and independence:

  • Assist patients with daily living activities, including bathing, dressing, grooming, and toileting
  • Support mobility, transfers, and basic exercises in alignment with the care plan
  • Help maintain a clean, safe, and comfortable home environment
  • Prepare meals and assist with feeding as needed
  • Take and record vital signs when required and trained to do so
  • Assist with medication self-administration as directed
  • Observe and report changes in patient condition to the clinical team
  • Document care provided accurately and timely within the EMR system
  • Participate in team meetings, trainings, and care coordination activities
  • Provide compassionate support to patients and families during each visit

  • Meets state and federal requirements for Home Health Aide certification or training
  • At least 18 years of age
