MDS Coordinator (RN)

Location: Austin, TX
Job Type: Full-Time

About the Opportunity

Our skilled nursing facility in Austin, TX is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to lead the Resident Assessment Instrument (RAI) process while ensuring accurate reimbursement, regulatory compliance, and exceptional resident care. This role works closely with the interdisciplinary team to optimize clinical outcomes and maintain compliance with CMS and state regulations.

Key Responsibilities

  • Coordinate and complete the MDS 3.0 assessment process in accordance with CMS and Texas regulations.
  • Ensure timely and accurate completion of all MDS assessments, Care Area Assessments (CAAs), and care plans.
  • Coordinate interdisciplinary care plan meetings and ensure individualized resident-centered care plans.
  • Monitor Medicare, Managed Care, and Medicaid reimbursement while maximizing case mix and reimbursement opportunities.
  • Review clinical documentation to ensure accuracy and support reimbursement.
  • Collaborate with nursing leadership, therapy, social services, dietary, and physicians regarding resident assessments.
  • Conduct resident assessments and participate in quality improvement initiatives.
  • Stay current with CMS guidelines, PDPM reimbursement methodology, and regulatory changes.
  • Assist with survey readiness and compliance initiatives.
  • Educate nursing staff regarding documentation requirements and MDS processes.

Qualifications

  • Current Texas Registered Nurse (RN) license in good standing.
  • Minimum 2 years of MDS Coordinator experience in a skilled nursing or long-term care setting preferred.
  • Strong knowledge of MDS 3.0, RAI Manual, PDPM, Medicare, Medicaid, and CMS regulations.
  • Experience with PointClickCare or other electronic health record systems preferred.
  • RAC-CT certification preferred but not required.
  • Excellent organizational, communication, and analytical skills.
  • Ability to work collaboratively with an interdisciplinary team.
