PRIDE Health is seeking a local contract nurse RN Home Health Hospice for a local contract nursing job in Springfield, Missouri.

Job Description & Requirements

Travel Registered Nurse (RN) โ€“ Hospice Case Manager

Location: Springfield, MO (50-mile coverage area)
Schedule: Mondayโ€“Friday | 8:00 AM โ€“ 4:30 PM
On-Call: 3 weekdays/week & 2 weekends/month
Productivity: 25 visits/week
Mileage Reimbursement: Yes

Job Summary

We are seeking a compassionate Travel RN โ€“ Hospice Case Manager to provide skilled end-of-life care for hospice patients in their homes. The RN will manage a patient caseload, perform nursing assessments, coordinate care with the interdisciplinary team, and provide education and support to patients and families.

Key Responsibilities
  • Manage hospice patient caseload and provide skilled nursing care.
  • Perform comprehensive patient assessments and develop individualized care plans.
  • Coordinate care with physicians and the interdisciplinary team.
  • Participate in IDG meetings every other Wednesday.
  • Provide patient and family education on symptom management and end-of-life care.
  • Complete accurate clinical documentation and maintain quality standards.
  • Participate in the on-call rotation as scheduled.
Required Qualifications
  • Active RN license
  • Current BLS certification
  • Valid driver's license and reliable transportation
Preferred Qualifications
  • Hospice experience
  • Homecare Homebase (HCHB) experience
  • Previous hospice case management experience

Dress Code: Business casual/scrubs as required.
