About the Opportunity:

The Registered Nurse (RN) - Home Health is responsible for providing skilled nursing care to patients in the comfort of their homes. The RN - Home Health works in collaboration with the interdisciplinary team, physician, and family to develop a comprehensive plan of care to meet the patient's needs. As a PRN (as-needed) position, the RN - Home Health will be called upon to work on an as-needed basis, based on the needs of the organization.

Responsibilities:

  • Ensure patient safety and provide high-quality nursing care
  • Assess the patient's condition and implement the care plan in collaboration with the interdisciplinary team
  • Provide wound care, medication management, IV therapy, and other skilled nursing services
  • Educate the patient and caregiver on disease management, prevention of complications, and self-care
  • Collaborate with physicians and other healthcare professionals to develop and implement the plan of care
  • Complete documentation of patient care in an accurate and timely manner
  • Participate in quality assurance and performance improvement activities
  • Attend and participate in interdisciplinary team meetings, as needed

Qualifications:

  • Active Texas RN license
  • At least 1 year of nursing experience, with Home Health experience preferred
  • Ability to work independently, with minimal supervision
  • Strong clinical and critical thinking skills
  • Excellent communication and interpersonal skills
  • Reliable transportation
  • Ability to work on an as-needed basis

Physical Demands:

The RN - Home Health must be able to perform physical activities that require considerable use of arms, legs, and moving the whole body, such as lifting, balancing, walking, stooping, and handling of materials. The RN - Home Health must also be able to navigate different home environments and adapt to various indoor and outdoor conditions.
