Permanent Medical Director Geriatric Medicine

Position Type: Permanent
Specialty: Physician Geriatric Medicine
Location: Indiana.
Shift: Full time, day shift

Position Details:
  • Patient Population: Frail/Elderly (PACE participants 55+)
  • Setting: PACE (Program of All-Inclusive Care for the Elderly) outpatient/center-based care
  • Responsibilities: Provide medical direction and supervision of participant care, oversee clinical outcomes and QAPI, supervise PCPs and APPs, conduct H&Ps and reassessments, develop treatment plans, lead interdisciplinary team collaboration, participate in intake/assessments and family conferences
  • Required Procedures: Physical examinations, chronic disease management, ordering/evaluating labs and diagnostics
  • EMR System:
  • Support Staff: Interdisciplinary team including PCPs, APPs, nurses, therapists, social workers
  • Days Per Month: Full time
  • On-Call/Weekends: No call, No weekends

Submission Requirements:

  • Board Certified in Internal Medicine or Family Medicine
  • Board Certified or Fellowship/CAQ in Geriatric Medicine
  • Active Indiana license (or willing to obtain)
  • NPDB Self Query within 30 days
  • Minimum 1 year experience with frail/elderly population
  • Supervisory experience
  • DEA, CDS, BLS (by start)
