Department: Social Services Job Status: Full-Time Shift: 8:00 AM - 4:30 PM Rate of Pay: $28.00 - $30.00 Hourly Position Type: Full-Time Status: Offer
Position Description
Are you passionate about helping individuals successfully transition back into the community? As a Re-entry Specialist, you'll work directly with individuals served to plan for discharge, coordinate community resources, and reduce recidivism. This is a meaningful opportunity to guide individuals toward independence, stability, and a successful return to the community.
What You'll Do
In this role, you'll serve as the primary point of contact for community agencies, coordinate aftercare referrals, and schedule follow-up appointments. You'll assess needs for benefits, medications, and housing, assisting with applications and ensuring continuity of care. You'll also help individuals navigate legal requirements, such as future court dates and registration obligations, and provide individual and group interventions to support successful reintegration. Throughout, you'll demonstrate cultural and age-specific competency while collaborating with treatment teams to promote long-term success.
What We're Looking For
  • Education: Bachelor's degree in social work, counseling, or a related field (required)
  • Experience: Two (2) years of relevant experience (preferred)
