Hours: Full time (37.5 hours per week), Day and Night Shifts available, 12.5-hour shifts, 3 days per week, with every other weekend rotation
Location: multiple locations surrounding Lynchburg, VA 24501
$5000 Hiring Bonus for Housing LPN, active as of 01/21/2025, $5000 Total, paid in three (3) installments; $2000 upon receipt of the following: Background clearances, including fingerprint background check, Acceptable drug screen results, Satisfactory references; $1500 upon satisfactory completion of the six (6) month probationary period; $1500 upon completion of twelve (12) months of continuous employment, with a satisfactory performance evaluation. This hiring bonus is applicable to new hires only, or otherwise, individuals not having been employed for a minimum of 1 year from the date application is received.
Position Summary:
The Licensed Practical Nurse (LPN) at Horizon Homes will manage all medical issues with direction from the RN and Physician and ensure physician's orders are implemented while keeping the physician and supervisor informed of pertinent information concerning the client's response to treatment.
Required Education and Experience:

  • The successful completion of a course of study that allows for licensure as a Licensed Practical Nurse (LPN) in the state of VA is required.
  • One year of experience providing nursing services as an LPN is required or an equivalent combination of training and experience that provides the required knowledge, skills, and abilities. Preference may be given to candidates with prior experience working with individuals with developmental disabilities.

Related Knowledge, Skills and Abilities:
Knowledge of the principles, procedures, techniques, trends, and literature related to physical health and intellectual disabilities; Knowledge of nursing best practices; Skill to administer medications; Skill to produce, read, and relay medical lab results; Ability to respond quickly to correct situations deemed to be hazardous; especially to consumers; Ability to record data and complete documentation within established expectations; Ability to demonstrate effective communication skills: verbally, person-to-person and through use of the telephone, and written; Able to train staff to implement training programs for the individuals; Commitment to serving the ID/DD population. Ability to lift and transfer consumers up to an average of 50 to 80 pounds; Safely assist individuals with all aspects of physical management including equipment use, safe lifting and physical assistance; Willing and available to work modified schedules (weekends, holiday, etc.) as required; Ability to drive and must possess a valid driver's license; Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites; Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed. May be called upon to travel to any Horizon worksite; transport clients, provide services to clients at other locations.
