As a Nurse Practitioner (NP) or Physician Assistant (PA) at CenterWell Senior Primary Care, you will be part of a collaborative, versatile team. Your Role & Responsibilities

  • You will provide comprehensive care to seniors following clinical standards.

  • You will collaborate daily with your care team to ensure coordinated, high-quality care.

  • You will manage referrals, hospital/SNF coordination, DME, and home health services.

  • You will document care with support from quality-based coders.

  • You will participate in on-call rotation and support growth plans at your center.

  • You will have experience providing value-based care.

What We're Looking For

Required:

  • You will have a Master's Degree in Nursing or completion of a PA program with board certification.

  • You will have an active, unrestricted NP or PA license in the state of practice.

  • If selected for this role, we will require you to screen for TB.

Additional Information:

  • You'll report to the Area Medical Director

  • This is an in-person role.

  • You will support 3 of our outpatient clinics in the area.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
