Description
This position is responsible for providing clinical support to providers and working in a patient-centered medical home to provide quality care to patients.

ESSENTIAL JOB DUTIES
  • Greets and rooms patients in a professional and welcoming manner.
  • Performs patient work-up which includes gathering reason for visit, patient health history, demographic information, assisting providers (physician, advance practice provider, clinical pharmacist, behaviorists) in performing medication reconciliation, verifying allergies, documenting vitals, and assisting patients with completion of screening questionnaires/forms.
  • Communicates expected wait times to patients and providers, providing updates frequently.
  • Participates in pre-visit procedures including scrubbing all patient charts for the day, verifying historical labs, updating lab results, obtaining medical records, as needed, and huddling with the provider.
  • Scrubbing for and addressing overdue health maintenance and gaps in care to assist in providing quality care and achieving quality metrics.
  • Prepares exam rooms by setting up appropriate equipment according to visit type, stocks, preps and disinfects exam rooms in between visits.
  • Autoclaves instruments and equipment after each use according to NHC standard procedures and protocol.
  • Accurately and timely inputs patient information into EPIC per NHC standard procedures and protocol.
  • Performs procedures within scope of responsibility, as directed by provider following standard protocol.
  • Performs phlebotomy for lab draws, as directed by provider following standard protocol.
  • Provides patient with after-visit summary (AVS), verbally reviews the information, and ensures all questions are answered and next steps in care are understood.
  • Answers phones with professionalism to collect and record patient information, schedule appointments, verify insurance, and patient information.
  • Responding to new or refill prescription requests through EPIC.
  • Conducts patient follow-up, including referral paperwork and assisting patients in scheduling additional medical tests or appointments.
  • Communicates lab results as directed by the provider either by letter or phone.
  • Scans and indexes documentation into medical records according to Clinic standard protocols and procedures.
  • Receives, documents, and routes messages to appropriate clinical staff, including responding/returning patient/caregiver phone calls to obtain missing or additional information.
  • Actively participates in continuous process and quality improvement workflows.
  • Attends and participates in committees and assists at other clinics as business needs requires.
  • Welcoming new team members and supports with training and development.
  • Comply with Clinic standardized workflows, protocols, HIPAA, and safety guidelines.
  • Performs other duties as assigned.


SKILLS | EDUCATION | CERTIFICATIONS
  • Completion of an accredited medical assistant program
  • One year of related experience and/or training
  • Experience in primary care preferred
  • Previous EPIC or EHR experience required
  • Certification through a nationally recognized Medical Assistant Certification program, such as AAMA, NCCT, or CCMA
  • Responsible for maintaining an active and unencumbered medical assistant certification and BLS
  • Ability to fluently speak, read, and write in Spanish is highly preferred
