Meals on Wheels Orange County is the largest nonprofit provider of nutrition and supportive services for at-risk older adults in Central and North Orange County. Our mission is to combat hunger and loneliness among older adults through nutritious meals, friendly visits, and safety checks, and to support families through day services. We deliver nearly 2 million meals annually to 20,000 at-risk older adults.

Location: Santa Ana, CA

Hours: Monday - Friday, 8:00 AM - 4:30 PM

Hourly Range: $23.00 - $28.00 per hour (Hourly rate will be determined based on factors such as geographic location, skills, education, and experience)


Make a Difference in the Lives of Older Adults and Individuals with Disabilities

Are you passionate about helping others navigate healthcare and community resources? We are seeking a compassionate, organized, and driven Care Coordinator Specialist II (CCS II) to join our team. In this role, you will work directly with older adults, individuals with disabilities, caregivers, healthcare providers, and community partners to ensure members receive the support and services they need to live safely and independently.

As a Care Coordinator Specialist II, you will manage a caseload of members, conduct assessments, coordinate care plans, facilitate referrals, support care transitions, and advocate for members in a variety of settings, including homes, hospitals, clinics, and community locations.

What You'll Do:

  • Manage member caseloads and conduct intake assessments and reassessments.
  • Coordinate care among physicians, healthcare providers, caregivers, and community organizations.
  • Screen members for service eligibility and connect them to resources including housing, mental health, transportation, employment assistance, and crisis services.
  • Develop, monitor, and update individualized care plans.
  • Support members through hospital discharges, care transitions, and specialty referrals.
  • Conduct home visits, hospital visits, and community-based outreach.
  • Accompany members to medical appointments as needed.
  • Maintain accurate documentation, case records, reports, and program statistics.
  • Represent the organization at community events, partner meetings, trainings, and networking opportunities.
  • Collaborate with interdisciplinary care teams to improve health outcomes and continuity of care.
  • Meet program goals, quality standards, and key performance metrics.

Qualifications Required:

  • Bachelor's degree (required) in Social Work, Social Services, Gerontology, Health Sciences, or a related field;
  • Minimum two (2) years of experience in case management, enhanced case management, care coordination, or care transitions.
  • Minimum two (2) years of experience working with older adults, seniors, individuals with disabilities, or other vulnerable populations.
  • Bilingual in Spanish or another threshold language preferred.
  • Strong verbal, written, and interpersonal communication skills.
  • Proficiency in Microsoft Office (Word, Excel, Outlook).
  • Ability to maintain confidentiality and professional boundaries.
  • Strong critical thinking, problem-solving, and organizational skills.
  • Valid driver's license, reliable transportation, and the ability to travel throughout the community as required.
  • Successful completion of a background check.

Preferred:

  • Experience with Care Transitions programs and methodologies.
  • Knowledge of Orange County senior and disability service networks.
  • Experience developing reports, program materials, and administrative processes.

Physical Requirements:

  • Regularly sit, stand, walk, and travel between community locations.
  • Frequently operate computers, phones, and office equipment.
  • Ability to communicate clearly and effectively in person, by phone, and electronically.
  • Frequently reach, extend arms, and perform repetitive hand movements.
  • Occasionally navigate stairs and various community environments.
