Advanced Practice Provider (APP) - Mechanical Circulatory Support (MCS) / LVAD

Position Summary
The Division of Cardiovascular Medicine at the Hospital of the University of Pennsylvania is seeking a full-time Advanced Practice Provider (APP) to join the Mechanical Circulatory Support (MCS)/Left Ventricular Assist Device (LVAD) program. This role supports a high-acuity, rapidly growing advanced heart failure and MCS service and functions within Penn Medicine's integrated, multidisciplinary care model.
The APP provides comprehensive outpatient care to patients supported with LVADs and to patients with advanced heart failure undergoing evaluation for mechanical circulatory support. The position requires progressive clinical independence, strong clinical judgment, and close collaboration with physicians, nursing, and interdisciplinary team members.
Role & Scope of Practice
The APP practices under the Penn Medicine Medical Staff Bylaws, within delineated clinical privileges, and in accordance with collaborative/supervisory agreements. Clinical responsibilities are defined by the MCS/LVAD leadership team and aligned with the Penn Advanced Practice role framework.
This role includes independent clinical decision-making while remaining fully integrated into a physician-led, team-based model of care.
Essential Functions
  • Provide independent outpatient evaluation, management, and follow-up for LVAD and advanced heart failure patients
  • Perform longitudinal management including medication optimization, symptom assessment, and device-related evaluation
  • Manage patient communications including triage, prescription management, and care coordination
  • Facilitate transitions of care across inpatient, outpatient, and post-acute settings
  • Collaborate with physicians, VAD coordinators, pharmacists, nursing, and allied health professionals
  • Contribute to quality improvement initiatives, clinical pathway development, and program operations
  • Support Penn Medicine's missions of clinical excellence, education, and research

Reporting Structure
This position follows Penn Medicine's dual-reporting APC model:
  • Administrative reporting to the Cardiology Advanced Practice Manager and VAD Manager of Clinical Operations for role oversight, performance evaluation, and professional development
  • Clinical reporting to MCS/LVAD Program Physicians, VAD Program Clinical Operations Manager, and Program Leadership for day-to-day clinical practice and service-line integration
  • The APP is accountable to both administrative and clinical leadership and to the broader service line and organization.

Credentials:
  • Education (one required):
Master of Science in Nursing (MSN), Adult-Gerontology Acute Care Nurse Practitioner
Master's degree in Physician Assistant Studies
  • Licensure & Certification:
Current CRNP or PA-C licensure in the Commonwealth of Pennsylvania
National certification as an Acute Care Nurse Practitioner or Physician Assistant
CPR and ACLS certification required
  • Experience:
Prior RN, NP, or PA experience in heart failure and MCS/LVAD strongly preferred
Prior NP or PA experience, ~ 2 years experience as an APP preferred
  • Credentialing:
Appointment is contingent upon review and approval by the Credentials Committee and Medical Board prior to start of employment
