Job description

Matthew Walker Comprehensive Health Center (MWCHC) has an exciting opportunity available for a skilled, resourceful and motivated individual who would like to make a difference in the lives of others in helping a non-for-profit organization fulfill its mission to provide quality health care. The Licensed Professional Counselor(LPC)/Licensed Clinical Social Worker(LCSW) will have the responsibility of providing counseling (talk therapy) for Matthew Walker Comprehensive Health Center (MWCHC) patients with mild to moderate mental health conditions, including trauma informed care (i.e PTSD, sexual assault), and/or substance use disorders. Additionally, responsible for implementing community educational programs, assessing problems and determining appropriate types and methods of treatment, and implementing intervention methodologies for mental health and substance abuse. The LPC/LCSW will utilize skills to address other social needs of patients and will utilize area referral sources for needed specialty care, hospitalization, or advance psychiatric assessments and treatments. This position is also responsible for behavioral health consultations with staff members regarding the mental health welfare of MWCHC patients. Additional responsibilities include, but are not limited to:

  • Providing counseling (talk therapy) for Matthew Walker Comprehensive Health Center (MWCHC) patients with mild to moderate mental health conditions, including trauma informed care (i.e PTSD, sexual assault), and/or substance use disorders
  • . Respond to request for consultations from physicians.

  • Assess the mental and behavioral health status of referred patients.

  • Provide counseling/talk therapy to scheduled patients utilizing professional counseling standards.

  • Formulate differential diagnosis of mild to moderate mental health conditions and/or substance abuse.

  • Serve as a liaison to the community.

  • Identify psychosocial needs of patients and make appropriate referrals.

  • Formulate short-term and long-term counseling plans for patients under care.

  • Provide written documentation (via Electronic Health Records) of counseling sessions, assessments, interventions, and recommendations for MWCHC’s providers and to meet requirements for medical records documentation.

  • Provides group and family therapy session as indicated.

  • Participate in a multidisciplinary team approach to patient care/treatment.

  • Support community health/behavioral health services during community visits on the Healthcare for Wheels for Everyone (H.O.W.I.E.) mobile health bus.

  • Assist in the assessment, planning, arranging, and implementation of psychosocial programs.

Current licensure for a Licensed Professional Counselor or Licensed Clinical Social Worker in the State of Tennessee. MS in field of Counseling or Social Work. Experience in an ambulatory health center preferred. Familiarity with multicultural groups, previous work experience in a multicultural setting and willingness to enroll language and multicultural coursework to supplement training if necessary.
