Salary: $55,000 - $59,000 annually

Since 1985, Albuquerque Health Care for the Homeless has dedicated its mission exclusively to providing services to people experiencing homelessness. AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. Our vision is to live in a world that is just and without homelessness. We believe that homelessness is an issue that can be solved through access to quality health care, adequate and affordable housing, and a living wage.


The Social Services Coordinator is responsible for the development, implementation, and oversight of the administrative and field operations of the Community Support Workers (CSW).


Duties and Responsibilities:


  • Monitor day-to-day activities and productivity of the teams
  • Provide training, performance review, coaching, support, and professional development to the teams
  • Ensure caseloads are covered by providing CCSS services as needed to bridge service gaps during staff absences or vacancies.
  • Coordinator will have direct service expectations to average 10 encounters per month
  • Report accurate and thorough statistical and narrative program data to other AHCH leadership, Board, and funders through Program Review Committee
  • Represent AHCH in meetings and trainings with community partners
  • Maintain adherence to all contractual and performance requirements, including timely and accurate reporting for relevant funders
  • Ensure quality assurance and quality improvement in all CSW activities including client services, billing, and documentation.
  • Work in an integrated manner with all AHCH departments to ensure access to wrap-around services for AHCH clients
  • Conduct program evaluation to determine opportunities to enhance client services and identify where barriers to client services exist within the programs
  • Work closely with the Behavioral Health Director and Social Services Director to coordinate clinical supervision of Community Support Workers (CSWs) in compliance with Comprehensive Community Support Services (CCSS) standards
  • Track billing data and effectively communicate impact of program performance on financial sustainability of the organization
  • Serve as a resource to other departments regarding social CSW programming
  • Track program budget in conjunction with Social Services Director
  • Maintain proficiency in the EHR
  • Facilitate appropriate and required training to team
  • Oversight on adherence to HIPAA compliance in the department
  • Will attend Program Review Committee meetings
  • Will attend Manager/Coordinator meetings


Minimum Qualifications:


  • Ability to thrive in a complex non-profit healthcare setting
  • Ability to work with peers to lead change, high performance, and innovation
  • Must have ability to analyze data and effectively communicate impact of data to AHCH leadership and staff
  • High level of organization and adherence to deadlines
  • Ability to work in an integrated manner with all levels of organizational staff
  • Previous experience working with underserved populations and/or those with SMI/CMI and/or substance abuse disorder
  • Bachelors degree in Human Services
  • 4 years of experience in social services or behavioral health setting working with AHCH target population
  • 1 year supervisory experience

Preferred Qualifications:


  • Masters degree in Human Services, Behavioral Health, or relevant field or commensurate relevant work experience
  • Previous experience in Medicaid billing and/or CCSS
  • CCSS certification
  • Previous experience working in a FQHC or community healthcare setting
  • Bilingual Spanish/English
