The ideal candidate will have prior experience in a Skilled Nursing Facility (SNF) or Long-Term Care setting and possess strong knowledge of admissions and discharge planning.

Licensed Master Social Worker Summary

The Licensed Master Social Worker will assist residents and families with emotional, social, and transitional needs while ensuring a smooth admission and discharge process. This role requires excellent communication, organizational skills, and the ability to collaborate effectively with interdisciplinary healthcare teams.

Licensed Master Social Worker Responsibilities
