Job description

Our client, located in western Massachusetts, is seeking a Social Worker/Behavioral Health Consultant for a Federally Qualified Health Center. The ideal candidate has a master's degree in social work with 1+ years of behavioral health experience. A LICSW or LMHC is needed. This is full-time, majority remote, 1 day a week onsite working part-time 32 hours.ย 


ย Essential Duties and Responsibilities:

  • Will advocate and help clients gain access to resources while addressing their personal issues, such as mental illness, addiction, and abuse.
  • Support the mission, vision, and values and will adhere to compliance protocols as well as policies and procedures.
  • Work closely with all members of the healthcare team in development of the Plan of Treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
  • Develop and maintain thorough knowledge of referral resources throughout the service.ย 


Minimum Requirements:

  • Masterโ€™s degree in Social Work
  • License: Professional license per state law
  • Experience: One (1) year full-time
  • LICSWย or LMHC
