Job description

POSITION SUMMARY
Capital Area Health Network (CAHN) is seeking a Licensed Practical Nurse (LPN) to aid in facilitating our Cessation Education and Social Environment (CEASE) Initiative. The Cease Initiative is a one-year pilot program with potential to extend an additional year to meet the prominent need of smoking cessation support in the Richmond City Community and surrounding counties. CAHN is looking for a LPN to support patients through the program. The LPN will report directly to the Clinical Coordinators, Clinical Managers and Chief Nursing Officer.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
  • Triage patients who present with interest in smoking cessation information, resources, and/or enrollment in CEASE program.
  • Evaluate referred patients for readiness to quit smoking and potential/current barriers to successful cessation.
  • Coordinate care and resources with the interdisciplinary team for clinical patient flow ensuring patient safety and providing solutions to any challenges and/or obstacles to desired goals in the program.
  • Establish daily, weekly, and monthly goals with enrolled CEASE Program patients.
  • Conduct health/educational assessments and re-evaluate daily, weekly, and monthly goals.
  • Facilitate in-person individual/group sessions as scheduled per team.
  • Travel to site(s), when necessary, to provide direct patient care, triage, and/or other duties as assigned.
  • Participate in case conferences with the interdisciplinary team and provide supporting facts where necessary
  • Assist the physicians with individual health education as directed and provide literature accordingly.
  • Assist with the development of policies, procedures, and clinical protocols.
  • Conducts monthly quality assurance and appropriate par levels on all medications and medical equipment as assigned.
  • Prepares monthly, quarterly, and annual reports in a timely manner.
  • Assists with the development of orientation for new employees and serves as the nurse preceptor.
  • Assist the RN and DON with administrative duties as assigned.
  • Empower clients to make healthy decisions in an effort to improve the quality of lives.
  • Maintain accurate patient records in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
  • Process routine, urgent and Stat referrals
  • Attend team meetings and provide clinical updates where necessary, and prepare training material.
  • Performs other duties as assigned.
POSITION REQUIREMENTS
  • Must have a valid Virginia's driver's license, satisfactory driving record, and transportation to travel to our various medical offices.
  • Must be licensed to practice in the state of Virginia, current CPR (Healthcare Provider) certification required with a minimum of three years of nursing experience.
  • Must have previous experience as a Licensed Practice Nurse within a community health and/or public health setting.
  • Must have leadership skills and demonstrate knowledge, skills and critical thinking abilities.
  • Must be computer literate and have previous experience with computerized charting
  • Must be able to staff flexible hours with the ability to work 5:00pm-9:00pm on assigned weekday evenings and/or Saturday mornings 8:00am-12:00pm.
