<p><b>Internal Medicine /Family Practice Physician- Hospitalist Green Bay WI</b></p> <p><b>Part time or Full time</b></p> <p> </p> <p>We are looking for an experienced Physician to join our group in Green Bay, WI. We are seeking a compassionate provider to be our go-to person for ensuring the high standards of quality care that we provide and lead the team at our Rehab Hospital Program.</p> <p>This opportunity is full-time or part-time at an Inpatient Rehab Hospital in Green Bay, WI. This position offers excellent work-life balance with flexible starting and ending times. You will have APP support to assist with daily documentation. You will experience one of the most collegiate groups of people you will ever work with. Great opportunity for off week rotation. The physician is responsible for diagnosing and treating patients in a patient centered compassionate environment as a part of a cohesive interdisciplinary clinical team.</p> <p><span style="background:white"><b>Responsibilities:</b></span></p> <ul> <li><span style="background:white"><span style="tab-stops:list .5in">History and Physical Exams- Examine patients and their medical records, pertinent documents/imaging/labs/test</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Order and study diagnostic tests (blood tests, X-rays etc.)</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Review and approve rehabilitation care plans as needed </span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Actively participate in interdisciplinary team and patient care conferences </span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Diagnosis, management and treatment of acute episodic illnesses and stable chronic conditions, evaluation of current therapies, health maintenance, health promotion, medications</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Write referrals and consult other physicians for comprehensive patient care</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Maintain accurate records and schedules- Establishes and maintains a system for tracking and coordinating the designated patient caseload and their progress throughout the entire acute or chronic episode of care</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Inform patients about their health conditions and prescribed medications</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Educate patients and their family members on diseases, epidemics and prevention- teaching and counseling to patients and families</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Collaboration with the healthcare team- Monitors, evaluates, and communicates relevant data regarding clinical problems consulting physicians and appropriate members of the interdisciplinary team</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Oversee patients’ overall health- identify opportunities for each patient's needs relative to his/her age, Monitors progress of patients and collaborates with other team members to ensure continuity of care.</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Identifies and addresses potential deterrents to compliance with treatment regime.</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Advance professional knowledge and conduct research</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Follows up with patients if they are discharged to assess if needs are being met and determines if additional follow-up care is style="tab-stops:list .5in">ACLS Certified</span></span></li> <li><span style="background:white"><span style="tab-stops:list .5in">Hospitalist experience preferred</span></span></li> </ul>
