Registered Nurse โ€“ Case Management

Schedule: Mondayโ€“Friday, 8:00 AMโ€“5:00 PM
Hours: 40 hours guaranteed
Coverage Area: Clayton, Fayette & Coweta Counties
Mileage: $0.49/mile

We are seeking a Registered Nurse to provide comprehensive case management and skilled nursing support for high-risk patients. The RN will assess patient needs, coordinate individualized care plans, facilitate transitions of care, and collaborate with healthcare providers to promote positive outcomes.

Responsibilities

  • Conduct comprehensive assessments of high-risk patients and develop individualized care plans.
  • Coordinate care with physicians, social workers, discharge planners, and other healthcare professionals.
  • Monitor patient progress and identify appropriate, cost-effective levels of care.
  • Coordinate transitions of care and connect patients with community resources such as transportation and housing support.
  • Provide skilled nursing services, including IV therapy, lab draws, wound care, wound vacs, Foley catheter insertion, PleurX drain care, and trach care.
  • Maintain detailed clinical and patient records.
  • Identify opportunities for health promotion and disease prevention.
  • Advocate for patient privacy, safety, and continuity of care.

Requirements

  • Active, unrestricted RN license; compact licenses accepted.
  • OASIS and HCHB experience required.
  • Home health experience .
  • Experience with IVs, lab draws, wound vacs, Foley catheters, PleurX drains, trach care, and wound care.
  • 30 productivity points required.
  • Associate or Bachelorโ€™s degree in Nursing or related field.
  • Case Management certification /required.
