Job description

IU Health Medical Group- Metro Region is seeking an Advanced Practice Provider to join our Primary Care team in Avon, IN. Consider joining an experienced team located within the Professional Office Center and in a fast-growing community. Here you will help manage the care of a diverse set of patients.
Position Details:
  • Full-time, Day Shift
    • Offering a 5- or 4-day work week
    • No weekends, holidays, or call, allowing for work/life balance
  • 100% outpatient
  • Expected patient volume is approximately 60-80 per week; Diverse patient population of all ages
  • Team includes 6 physicians and 2 other advanced practice providers
  • Onsite Pharmacy support
  • Located on the campus of IU Health West Hospital
Key Responsibilities:
  • Assess health status through obtainment of relevant health history and comprehensive physical exam, preventative screening, and identification of medical risk factors.
  • Formulate appropriate differential diagnosis and identification of individual, family, and community needs.
  • Develops treatment plan to maximize the health potential of the individual, family, and/or community.
  • Orders and adjusts medication per collaborative written agreement and prescriptive authority in accordance with the Indiana Nurse Practice Act, or in consultation, and with signature of physician.
  • Serves as a clinical resource for the nursing staff, physicians, hospital, and other healthcare providers.
  • Serves as an advocate for patients, staff, and management.
  • Manage acute and long-term care of the patient.
Qualifications:
  • Master's degree required
  • FNP or PA certification required
  • 1-2 years of outpatient experience in primary care or urgent care highly preferred
  • Will consider highly motivated new graduates with ICU, ER or Critical Care RN Experience
