Position Overview:

Case management support to individuals with a history of serious and persistent mental illness. The successful incumbent will work in partnership with clients, families and the clinical team. They will meetย with clients in their homes, in the community or in the office. They will set individualized care plans with the client to improve functioning with activities of daily living; provide skills teaching; facilitate connections to community resources

and aid with navigating medical and social assistance systems. Support is dependent on the clients' specific needs and goals.

Key Qualifications:

  • Bachelor's Degree or Community College degree in a health-related discipline
  • Current registration with a recognized college
  • Minimum 3-5 years' experience working with adult clients living with severeย 
  • mental ย  illnessย 
  • Knowledge of psychosocial rehabilitation and recovery philosophy
  • Working knowledge of community resources
  • Excellent communication, organization, presentation and problem-solving skills
  • Valid driver's license, relevant insurance and use of own vehicle
  • Demonstrated ability to work independently and as part of a team
  • Treatment planning and intervention experience required
  • Group facilitation skills
  • Experience working with clients with substance misuse issues is an asset
  • Proficient computer skills
  • Reliable, flexible and positive team player
  • Positive attendance records are essential
  • Ability to speak a second language is an asset
