Summary

Familiar with functions of HIM Clerk and other HIM Technician positions to accurately direct calls for information and assistance. Responds to requests for patient records and patient information for continuation of care, court and other purposes outlined in procedure.Major Job Functions

The following is a summary of the major essential functions of this job. The incumbent may perform other duties, both major and minor, that are not mentioned below. In addition, specific functions may change from time to time:

  • Receives and responds to medical records and record information requests from authorized persons/facilities

  • Utilizes various Master Patient Indices (hard-copy and electronic) to accurately locate any prior patient visit history for logging requests for release of information

  • Processes all interoffice and incoming mail and routes information timely and accurately

  • Accurately updates the chart tracking system in responding to requests for release of information and chart sign-out

  • Notifies staff of location and availability of medical records for continuation of care and review

  • Other duties as assigned

Minimum Qualifications

The following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:

Education and Formal Training:
  • Associate's degree in related field required

Work Experience:
  • 2 years office experience in acute care Health Information Management or equivalent Health Information Management technical training required

Knowledge, Skills, and Abilities Required:
  • Proficiency in reading, writing, and speaking the English language

  • Computer literacy to include use of word processing, Windows, and/or mainframe computer systems

  • Excellent guest relations skills

  • Knowledge of medical terminology

  • Excellent written and oral communications skills, including English usage, punctuation and grammar

  • Types 45 wpm accurately

  • Ability to work under pressure with time constraints

Physical Requirements:
  • Flexibility in scheduling required

  • Visual acuity for reading and processing charts/information

  • Manual dexterity and motor coordination for processing patient records and accessing computer data

  • Hearing and speech to answer phones and greet public

  • Work is mostly sedentary

  • Ability to pull charts upon request for completion, patient care and release of information follow-up
