Responsibilities- Conduct thorough patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and communicate changes in condition to healthcare providers.
- Provide education and support to patients and their families regarding health management.
- Maintain accurate and timely documentation of nursing care and patient records.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Follow all safety protocols and maintain compliance with healthcare regulations.
Requirements- Valid Registered Nurse (RN) license in the state of Michigan.
- Previous experience in home health care or a related nursing field preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage a caseload effectively.
- Reliable transportation to travel to patient homes within Macomb, MI.
