Now Hiring: Nurse Practitioner or Physician Assistant - Endocrinology
Salary: $150,000-$220,000 annually
Elaya Health is a rapidly growing endocrinology practice specializing in providing high-quality care to patients in skilled nursing and long-term care facilities. We are seeking an experienced and compassionate Nurse Practitioner or Physician Assistant to join our expanding team.
This is an excellent opportunity for a provider looking for a high level of clinical autonomy, flexible scheduling, strong administrative support, and the opportunity to practice within a growing specialty group.
About the Role
Our providers deliver on-site endocrine care to residents of skilled nursing and long-term care facilities throughout New York and New Jersey.
You will manage patients with diabetes, thyroid disease, and other common endocrine conditions while working closely with facility nursing teams, primary care providers, and administration.
Responsibilities
  • Perform on-site patient evaluations and follow-up visits
  • Manage diabetes, thyroid disorders, and other common endocrine conditions
  • Review blood glucose trends, laboratory results, and medication regimens
  • Optimize diabetes management and help reduce unnecessary finger sticks and medication burden
  • Collaborate with nursing staff, DONs, facility leadership, and primary care teams
  • Provide clear clinical recommendations and ongoing follow-up
  • Document in facility EMRs, with training and administrative support provided
  • Help reduce avoidable hospitalizations through proactive specialty care

Qualifications

  • Strong clinical judgment and communication skills
  • Comfortable working independently while collaborating with a larger clinical team
