We are seeking a Hospice Clinical Liaison (LPN/LVN) to represent our hospice program to healthcare facilities such as nursing homes, assisted living communities, and hospitals. In this role, you will be the bridge between patients, families, and care teams—providing education, coordinating referrals, and ensuring a smooth transition to hospice services. This position requires clinical expertise, excellent communication skills, and a passion for building lasting relationships in the healthcare community.

Essential Functions

  • Educate referral sources on hospice benefits, clinical outcomes, and eligibility criteria
  • Meet with patients and families to explain hospice services and coordinate the admission process
  • Gather and submit clinical information to support patient terminal prognosis for RN review
  • Collaborate with branch staff, field sales teams, and referral sources to ensure timely care coordination
  • Participate in community outreach through speaking engagements, in-services, and healthcare events
  • Support compliance with federal, state, and Medicare hospice regulations
  • Document activities accurately in the contact management system (CRM)

Knowledge, Skills, and Abilities

  • Excellent interpersonal and customer service skills.

  • Ability to work independently while thriving in a team-focused environment.

  • Strong presentation and communication skills in both formal and informal settings.

  • In-depth knowledge of hospice regulations, discharge planning, and referral processes.

  • Proficiency with Microsoft Word, Excel, and CRM/documentation systems.

  • Willingness to travel within an assigned territory, with occasional overnight stays.

Education

  • Graduate of an approved school of Practical/Vocational Nursing.

Experience

  • Minimum two years’ experience as an LPN/LVN in hospice, home health, oncology, or long-term care.

  • Three to five years of clinical liaison or healthcare marketing experience preferred.

Licenses/Certifications

  • Current LPN/LVN license in the state of practice.

  • CPR certification.

  • Valid driver’s license and automobile insurance.


Compensation may vary within the salary range provided based on several factors, including but not limited to a candidate's geographic location, experience, education, skills, licensure, certifications, internal equity, and department equity. Gentiva provides associates with a comprehensive Total Rewards package, of which base pay is just one component.

Salary Range: $90,000 – $105,000
