Job Summary
We are seeking a Licensed Vocational Nurse to join our team. In this role, you are a Licensed Vocational Nurse (LVN) who operates with great attention to detail and a big heart. As a wound care LVN, you are responsible for providing skilled nursing care focused on the assessment, treatment, and management of acute and chronic wounds. You will monitor and document the patient's wound progress, reporting changes in condition to supervising RN or Provider.
Responsibilities
  • Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
  • Administer wound care treatments such as dressing changes and application of topical agents while maintaining aseptic technique and infection control standards.
  • Document any changes in patient conditions and report on changes
  • Maintain accurate records of patient care and discuss observations with the supervisor
  • Perform comprehensive wound assessments, including measuring, documentation, and evaluation of healing process using advanced medical terminology and physiology knowledge.
  • Document all patient interactions accurately within EMR/EHR systems such as WoundAlly, adhering to HIPAA regulations and medical documentation standards.
  • Educate patients and their families on wound management techniques, nutrition for healing, and lifestyle modifications to promote long-term health
  • Visiting patients home, facilities, etc. in the Los Angeles County, Orange County, Inland Empire and surrounding areas.
Qualifications
  • Currently Licensed Vocational Nurse (LVN)
  • Previous experience in healthcare is preferred
  • Excellent interpersonal and client care skills
  • Valid drivers license/Insurance
  • First aid/CPR certification required
  • Multilingual preferred
