He is wheelchair bound, limited speech, hemiplegic, neurological and/or behavioral health experience a must.

Available Shifts in Park City:
  • Night shifts from 7pm - 7 am. 2 or 3 nights per week, Saturday, Sunday, Monday
  • Afternoon shifts Mon - Fri, 3pm - 7pm
We are looking for an RN or LPN who is interested in the following:
  • Medication set up and oversight
  • Provide thoughtful care to clients based on personalized Plan of Care
  • Provide in home assessments
  • Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
  • Collaborate with client's family on plan of care, health changes, and concerns
  • Total personal cares

RN and LPN Requirements
  • Must have at least 1 year nursing experience with neurological and/or behavioral health
  • Must have at least 1 year nursing experience in home
  • Current unrestricted RN or LPN license in Utah
  • Certified CPR Certification
  • Negative TB skin test or chest x-ray
  • Must be able to pass a drug test and a background check
  • Licensed driver with valid automobile insurance
  • Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
  • Must be able to lift 75 pounds
