Licensed Practical Nurse (LPN)

Deer Creek Nursing and Rehabilitation is looking for an experienced Licensed Practical Nurse (LPN) to join the long-term care unit at our beautiful skilled nursing facility. Duties and Responsibilities

· Provide comprehensive Licensed Practical Nurse (LPN) care for residents

· Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols

· Monitor residents’ health status and promptly report changes in condition to the supervising RN, physician, and appropriate family members or responsible parties

· Provide skilled nursing services within scope of practice, including but not limited to tube feedings, suctioning, catheter care, wound care, dressing changes, and other ordered treatments.

· Accurately complete all required documentation, charting, and reports in accordance with regulatory requirements and facility policies.

· Support and implement individualized resident care plans while ensuring resident dignity, preferences, and rights are respected.

· Observe residents closely and report significant changes in physical, mental, or emotional condition to the interdisciplinary care team

· Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care

Schedule

· Schedule: Mon- Fri. 2p-10p and Double Weekend

· Location: Wimberley, Texas

Skills and Qualifications

· Active Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) license issued by Texas required

· BLS certification from the AHA required
