AM, PM, and NOC (Overnight) Shifts Available

Shift Differentials

  • PM Shift: +$2.00 per hour
  • NOC Shift: +$1.50 per hour

Sign-On Bonus

  • $750 after 3 months of employment
  • $1,500 after 8 months of employment

(Available for positions scheduled 56 hours biweekly or more.)

Position Summary

St. Paul Elder Services is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our nursing team. As an LPN, you will provide direct resident care while supporting Registered Nurses and collaborating with the interdisciplinary care team to deliver exceptional, resident-centered care.

This role includes medication administration, resident assessments, care planning, clinical documentation, staff support, and communication with physicians, residents, and families. Opportunities are available in Rehabilitation, Long-Term Care, and Memory Care.

Key Responsibilities
  • Provide compassionate, high-quality nursing care to residents.
  • Assist with the development, implementation, and revision of individualized resident care plans.
  • Understand resident diagnoses and help establish appropriate care approaches and goals.
  • Collect resident assessment data and report changes in condition to the Registered Nurse for follow-up.
  • Administer medications and treatments in accordance with physician orders and nursing standards of practice.
  • Perform complex nursing procedures under the supervision of a Registered Nurse after competency has been verified.
  • Review physician orders, medical records, and nursing care plans to ensure continuity of care.
  • Document resident conditions, treatments, and changes accurately and thoroughly.
  • Participate in interdisciplinary care conferences and contribute to resident care planning.
  • Supervise and provide direction to Nurse Techs, Medication Technicians, and Certified Nursing Assistants (CNAs).
  • Assist physicians during treatments and physician progress visits.
  • Prepare work schedules and assignments for nursing support staff.
  • Complete staff competencies and provide instruction on nursing techniques and procedures under the supervision of a Registered Nurse.
  • Monitor the safe use of medical equipment and supplies.
  • Educate residents and family members regarding care and treatment plans when appropriate.
  • Report significant resident changes or facility emergencies to nursing leadership.
  • Participate in Quality Assurance and Performance Improvement (QA/QI) initiatives.
  • Collaborate with all departments to promote exceptional resident care and positive outcomes.
Required Qualifications
  • Current Wisconsin Licensed Practical Nurse (LPN) license.
  • Strong clinical assessment and organizational skills.

  • Ability to provide compassionate, resident-centered nursing care.
