Job description

Louisiana Health and Rehab Center is seeking a Licensed Practical Nurse to join our team!


Summary: LPN to perform nursing duties in a residential setting, Part-Time, PRN, and on-call after hours and weekends. Will assume responsibility for direct care of assigned patients under the supervision of a registered nurse and/ or physician. The LPN will utilize knowledge of patients' treatment plan, while providing quality nursing care through techniques that support and achieve positive outcomes in patient services.


Job Duties include but not limited to:


1. Medication management of client prescription refills

2. Complete medical assessments upon client admission.

3. Assist in the discharge preparation process.

4. Ability to properly chart, document, and maintain patients' medical records.

5. Ability to observe and monitor the patient's behavior, mental status, and activities.

6. Maintain records of clients' vital signs and routine screenings ( including blood pressure, pulse, respiratory, and urinalysis).

7. Participate in individualized treatment plan, multi-disciplinary team meetings and conduct trainings as needed.

8. Strive to create a safe, healing environment for patients.

9. Communicate with peers and management regarding any hazards identified.

10. Ensure proper documentation is complete and adheres to standard policies and procedures set forth by federal and state regulations.

11. Must be able to meet and pass pre/post employment requirements including criminal background and drug screening.

Perform other duties as assigned, related to the position.


Required Education:

Graduated from an accredited nursing program with a Licensed Practical Nurse Diploma.

LPN License must be in good standing with the State of Louisiana, or Valid Multi-State Compact Nurse License.



Must be able to pass a background check and Drug screening.
