About the Role:

Haven of Bridgeport is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our skilled nursing and long-term care team. As an LPN at Haven of Bridgeport, you'll have the opportunity to provide exceptional resident care in a supportive environment with consistent scheduling and opportunities for advancement.

Responsibilities:

  • Provide direct nursing care in a skilled nursing and long-term care setting

  • Administer medications, treatments, and IV therapy as prescribed

  • Assess, monitor, and document resident conditions and care plans

  • Collaborate with physicians, nurse practitioners, and interdisciplinary care teams

  • Supervise and support Certified Nursing Assistants (CNAs)

  • Ensure compliance with Illinois nursing standards and facility policies
