Job description


Role and Responsibilities

Administer practical nursing care to patients in accordance with physician’s orders under the direction and supervision of a Registered Nurse.

Essential Functions

Under the direct supervision of an RN Case Manager, and within the scope of practice for a Licensed Practical Nurse:

  • Provide health education according to the teaching plan established by the RN.
  • Provide routine LPN nursing care as outlined in the nursing care plan and within the scope of practice for an LPN.
  • Observe signs and symptoms, and report patient’s reactions to treatments including drugs and changes in the patient’s physical or emotional condition, to the physician and RN Case Manager.
  • Document any services rendered to the patient, changes noted in the patient’s condition, and/or family and home situations.
  • Document communication with RN Case Manager and physician.
  • Other duties as assigned.

Education Requirements

  • Associate’s degree required.
  • Diploma as a Licensed Practical Nurse from an accredited program.

Qualifications and Requirements

  • Current, valid LPN license issued from NYS Department of Education.
  • Experience in home health care preferred.
  • Community health experience preferred.

Work Environment

The LPN is primarily in a non-office setting and may be exposed to outdoor conditions.


The working conditions are classified as medium work:

  • Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
