Job description

Community Healthcaring KW provides comprehensive primary health care, including medical and dental services, health promotion, illness prevention, and community outreach. Through an interdisciplinary team of health care professionals, we deliver person-centred care that improves health equity, reduces barriers to accessing services, and addresses the social determinants of health to support the well-being of individuals, families, and communities across Waterloo Region.


We are currently recruiting for Registered Practical Nurse to provide services at Lancaster Medical. While this position is employed by Community Healthcaring Kitchener-Waterloo, the RPN’s day-to-day work location schedule of hours, as well as applicable policies and procedures, will be determined by the Lancaster Medical team to which they are assigned.

Registered Practical Nurse – Lancaster Medical

$27.95 -$33.68 per hour

FTE 0.6 Part Time, Permanent

22.5 Hours per week

Position Summary

The Registered Practical Nurse (RPN) provides comprehensive, patient-centred primary care and patient support within Lancaster Medical. Working to full scope of practice as part of an interdisciplinary team, the RPN delivers direct clinical care including assessments, vital signs, immunizations, chronic disease support, and health education. The RPN collaborates with physicians and other health professionals to improve and support better health outcomes for individuals, and families.

Specific Responsibilities

  1. Complete history of clients, anticipate and ensure equipment for client visit is set up in room.
  2. Triage client’s needs, either by telephone or in person.
  3. Screen and assess clients with minor episodic illnesses according to established protocols.
  4. Perform basic nursing skills (e.g., dressing of wounds, injections, suture /clip removal and assisting with other procedures as requested).
  5. Provides Health education/counselling and other clinical activities in accordance with the College of Nurses of Ontario standards of practice for Registered Practical Nurse and all relevant provincial legislation and regulations.
  6. Perform necessary clinical functions to facilitate delivery of comprehensive primary care and patient flow (e.g., assist providers with prescription ordering; clean equipment).
  7. Maintain accurate health records and up to date clinical notes in accordance with College of Nurses Standards and policies.
  8. Ensure all exam rooms are stocked sufficiently and consistently.
  9. Order medical supplies and maintain inventory controls.
  10. Maintain contact with pharmaceutical company representatives and maintain a current inventory of pharmaceutical samples.
  11. Organize printed resources such as client education materials, diagnostic and other requisitions.
  12. Develop and monitor operating budgets for assigned projects/programs and ensure expenditures remain within budget.
  13. Maintain a valid driver’s license, access to a reliable personal vehicle and adequate insurance for work-related travel in accordance with the Health Centre’s policies.
  14. Work during both regular and extended hours (evenings/weekends) as determined by the Health Centre & Lancaster Medical. A minimum of one evening per week will be required.
  15. Participate in team and staff meetings and other meetings.
  16. Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
  17. Work in a manner that preserves privacy and confidentiality.
  18. Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.
  19. Work in a manner that demonstrates self-reflection and personal accountability for work performance.
  20. Collect, analyze and report on data and relevant information as required.
  21. Participate in research as determined by the Health Centre.
  22. Adhere to all Health Centre and Lancaster Medical policies.
  23. Adhere to appropriate infection control practices.
  24. Perform any other duties as assigned by your supervisor.

Qualifications

  1. Current registration as a Registered Practical Nurse in good standing with the College of Nurses of Ontario.
  2. A minimum of 1-2 years employment experience, including in a primary. A combination of primary care, community and acute care experience will be considered.
  1. Knowledge of available resources accessible to clients (e.g. food banks, local shelters, mental services)
  2. Experience working with Vulnerable Population is considered an asset.
  3. Demonstrated ability to communicate effectively in English, both written and verbally.
  4. Second language is an asset.
  5. Competency in MS Office Suite programs. Experience with electronic health records and other databases is an asset.
  6. Ability to work independently as well as an effective member of an interdisciplinary team.
  7. Ability to work collaboratively.
  8. Ability to maintain strict confidentiality.
  9. Experience working sensitively with culturally and economically diverse populations.
  10. Experience in the Family Practice office is an asset for at least 2-3 years.
  11. Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
  12. 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.
