Job Title: Licensed Practical Nurse (LPN)Reports to: Unit ManagerOverview: Van Duyn Center for Rehabilitation and Nursing is a 513-bed skilled Nursing facility atop Onondaga Hill in Syracuse, NY. The Van Duyn team is committed to providing quality care and diversified services with a professional team of caregivers.Duties/Responsibilities:
  • Medication administration
  • Charge nurse role for oversight of CNA’s to monitor care provided
  • Monitor effectiveness of the patient/resident plan of care
  • Participate in the care planning process
  • Reporting to nurse manager/nursing supervisor/ADON
Requirements/Qualifications:
  • Valid LPN Practical Nurse License
  • CPR Certification
  • New Graduates are welcome to apply No Experience necessary
