Licensed Practical Nurse/LPN, Pool, Milwaukee, WI  (Sign-On Bonus Available)

Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. We are offering rewarding opportunities to provide care and service in peaceful, tidy and well-supplied residential and clinic settings. Pool LPN’s may also cover home care.

We offer our pool LPNs assignment opportunities on all shifts across our community-based settings primarily in Milwaukee County and occasionally at our sites in Racine and Waukesha counties. Our greatest need for coverage is on 2nd and 3rd shifts as well as weekends.

The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a pool opportunity on all three shifts and will provide coverage throughout all Community Care sites.

Licensed Practical Nurse/LPN Job Responsibilities:  

·       Work in collaborationwith other nursing staff to coordinate and provide direction of care during weekday operations.  

·       Serves as coordinator of care for scheduled weekend and holiday shifts.  

·       Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin. 

·       Requires travel to various sites as needed.

·       Conducts home visits as needed.

·       Must complete LPN competency checklist within required time line.

·       Completes all orientation and onboarding requirements per established timelines.

·       Must be able to complete online/in person competencies.

·       Must attend any mandatory required trainings.

Licensed Practical Nurse/LPN Job Requirements:  

·       Education:Diploma from accredited program in practical nursing required. 

·       Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing 

·       Certification: Current CPR/AED certification

·       Experience: Minimum of one year experience with older adults or persons with developmental and/or physical disabilities in a hospital, homecare or nursing home setting is required. One year of practical nursing experience is preferred.  

Licensed Practical Nurse/LPN Job Schedule: 

·       Tier 1: Minimum of 2 shifts per month, 1 of which is a weekend day. If no weekend day available, 2 week day shifts required. 1 major holiday each year required.

·       Tier 2: Minimum of 4 shifts per month, 2 of which is a weekend day. If no weekend day available, 4 week day shifts required. 2 holidays per year, 1 major and 1 minor.
