Duties and Responsibilities
Provide total nursing care to residents on the assigned unit during the shift as a Licensed Practical Nurse (LPN)
Participate in developing and implementing individualized care plans alongside the healthcare team
Administer medications and treatments as prescribed and evaluate effectiveness regularly
Maintain an environment that promotes residents' physical, social, and psychological well-being
Educate residents and families on care plans and medical procedures
Monitor team member performance and assist with development, evaluation, and corrective action if needed
Delegate and ensure completion of assigned nursing tasks to team members
Stay current with facility policies, procedures, and ongoing nursing education
Recognize and act on significant changes in resident conditions
Document care, responses, and relevant information per facility guidelines
Act as a charge nurse: answer questions, resolve issues, and support resident care decisions
Follow all facility safety policies and procedures
Skills and Qualifications
Graduation from an accredited nursing education program
Active Licensed Practical Nurse (LPN) license in the state of Illinois
No prior abuse convictions
Strong communication and teamwork skills
Ability to follow policies, maintain safety, and deliver empathetic care
Must be able to work with diverse resident populations, including elderly and mentally ill individuals
Must be physically able to perform nursing duties with or without accommodations
