Licensed Practical Nurse (LPN) Job Summary

Spring Creek Rehab, a leading provider of healthcare services, is seeking a dedicated Licensed Practical Nurse (LPN) to join our team in Murray, Kentucky. As an LPN with our organization, you will play a vital role in providing quality care to our patients.

Licensed Practical Nurse (LPN) Job Responsibilities

  • Provide direct bedside care to patients, including but not limited to vital sign monitoring, wound care, and medication administration

  • Educate patients and family members on what to expect when dealing with the challenges of aging or specific conditions, and encourage them to take an active role in their recovery and long-term wellness

  • Inform family members of any changes in the patient's condition and discuss with them any alterations to the patient's medication or other treatments

  • Collaborate with the facility physician to develop and implement patient care plans, and notify the physician of any changes in the patient's condition

  • Maintain accurate and up-to-date records of patient care and communicate effectively with other healthcare professionals
