Deliver skilled care where patients are most comfortable-at home.
We're looking for a compassionate and dependable Licensed Practical Nurse (LPN) who enjoys providing one-on-one patient care in a home health setting. This is an opportunity to build meaningful relationships with patients while delivering quality nursing care that helps them recover, manage chronic conditions, and remain safely at home.
What You'll Do
  • Provide skilled nursing care under the direction of a Registered Nurse and physician
  • Administer medications and treatments as ordered
  • Monitor patient conditions and report changes to the supervising RN
  • Reinforce patient and caregiver education on medications, treatments, and disease management
  • Assist with developing and following individualized plans of care
  • Complete timely and accurate clinical documentation
  • Collaborate with physicians, caregivers, and the interdisciplinary care team
