Job description

Crestview Nursing & Rehabilitation is actively hiring a Licensed Practical Nurse (LPN) to provide skilled nursing care and clinical support as part of our PRN pool. The Licensed Practical Nurse (LPN) plays a key role in resident care, treatment delivery, and monitoring clinical status. As a Licensed Practical Nurse (LPN), you will work closely with Registered Nurses and the care team to deliver high-quality resident care on an as-needed basis.

Schedule and Employment Type

  • Full-Time Days (6AM - 6PM)

  • Full-Time Nights (6PM - 6AM) | Every Other Weekend Required

Key Responsibilities

  • Provide skilled nursing care as a Licensed Practical Nurse (LPN) within scope of practice

  • Administer medications and treatments as ordered

  • Monitor resident conditions, vital signs, and treatment responses

  • Accurately document nursing care and observations

  • Communicate resident status changes to Registered Nurses and providers

  • Support and guide nursing assistants during assigned shifts

Qualifications

  • Current state licensure as a Licensed Practical Nurse (LPN)

  • Strong clinical skills and attention to detail

  • Ability to work effectively in a fast-paced clinical environment

  • Skilled nursing or long-term care experience preferred
